Transient regional osteoporosis of the hip (TOH) is an uncommon cause of hip pain. It affects mostly healthy middle-aged men, and women in the third trimester of pregnancy. The aetiology is unknown. Differential diagnosis includes avascular necrosis, stress fracture, septic arthritis, malignancy, soft-tissue injury, osteoarthrosis and radiculopathy. Due to its association with bone marrow oedema, TOH is a pathology in which magnetic resonans imaging is extremely important in differential diagnosis. In this paper, a male patient with complaint of hip pain and difficulty in walking, which was diagnosed as TOH was presented by reviewing current literature.
