Postmenopozal Osteoporozda Risedronat Tedavisinin Kemik Mineral Yogunluguna Etkisi Biyokimyasal Belirleyicilerle Önceden Belirlenebilir mi? - Orijinal Arastirma

ÖzetBu çalismada risedronatin postmenopozal osteoporozda kemik mineral yogunlugu (KMY) ve biyokimyasal belirteçler üzerine olan etkilerinin birbiriyle iliskisini degerlendirmek amaçlandi.Çalismaya 103 postmenopozal osteoporozlu kadin olgu alindi. Bu olgularin 58'i grup 1, 45'i grup 2 olmak üzere ikiye ayrildi. Grup 1'e günlük 5 mg risedronat ve 500 mg kalsiyum, grup 2'ye ise sadece günlük 500 mg kalsiyum verildi. Lomber vertebra ve femurun dansitometrik ölçümleri tedavi öncesi ve 12. ay sonunda yapildi. Ayrica grup 1'deki 33 hastanin 2. yilda KMY ölçümleri tekrarlandi. Tedavi öncesi, tedavi sonrasi 1. ve 3. aylarda serum osteokalsin ve idrar C telopeptit (CTX) düzeyleri degerlendirildi.Grup 1'de çalismanin 1. ayindan itibaren idrar CTX ve osteokalsin düzeylerinde anlamli bir düsüs saptandi (pTedavinin 12. ayinda grup 1'de femur boyun ve lomber vertebra KMY'leri anlamli olarak artmisti. 2. yil dansitometrisinde ise, femur trokanter bölgesinde de tedavi öncesine göre anlamli artis saptandi (pGrup 1'deki hastalarin tedavi sonrasi 1. ve 3. aylardaki idrar CTX, ve osteokalsin degisimleri ile 1. yil sonundaki dansitometrik degisimleri arasinda anlamli bir korelasyon saptanmadi (p>0.05). Fakat tedavi öncesine göre 2. yildaki ortalama femur trokanter bölgesindeki KMY artisi, 1. ayda ortaya çikan CTX düsüsü ile korale bulundu (pSonuç olarak, uzun süreli risedronat kullanilacak hastalarda, tedavinin daha 1. ayinda idrar CTX düzeyine bakilarak, ilerleyen yillardaki femur KMY artisi tahmin edilebilir. The patients were divided into two groups as group 1 and group 2. The patients in group I were treated with 5 mg risedronate and 500 mg calcium daily, while group 2 were treated only with 500 mg of calcium.Densitometric measurements of lumbar spine and femur were performed before treatment and at the end of 12 months after treatment. Additionally, BMD measurements of 33 patients in group 1 were repeated at the end of 2nd year. Serum levels of osteocalcin and urine C telopeptyde (CTX) levels were measured before therapy and at the 1st and 3rd months of the therapy period.Significant decreases were observed in urine CTX and serum osteocalcin levels beginning from the 1st month in group 1 (pIn group 1, BMD of femur neck and lumbar spine regions increased significantly at the 12th month of the study. Femur trochanter BMD was also found to increase when compared with pretreatment values at the end of the second year evaluation (pNo significant correlation of urine CTX and serum osteocalcin levels which were measured at the 1st and 3rd months of the therapy was found with the densitometric changes at the end of one year in group I (p>0.05). However increase of mean BMD values of femur trochanter at second year according to pre-treatment values correlated with decrease of urine CTX values at the first month (pAs a result, urine CTX levels measured at the end of the first month of the treatment can help estimate the femur BMD increase in the following years of long term risedronate treatment. Klasik olarak osteoporoz patogenetik açidan yeniden yapilanma döngüsünde yikimin yapimi astigi yani eslesme fenomeninin bozuldugu ve net kemik kaybinin gelistigi kosul olarak tanimlanir (1,2). Kemik döngüsü, her yeniden yapilanma ünitesinde osteoblastlarin yaptigi formasyon ve osteoklastlarin yaptigi rezorbsiyon ile birbirine karsi olan bir aktiviteyle gerçeklesir. Kemik kütlesi, bu yapim ve yikim arasindaki dengeye baglidir. Menopozda pek çok kadinda kemik döngü hizi artar, kemik yikimi kemik yapimini asarak kemik kaybina neden olur (3).Kemik yikimini azaltan antirezorptif ilaçlar, kemik yikimi ile kemik yapimi arasindaki dengesizligi azaltarak, kemik döngüsü hizini düsürerek yada her iki mekanizmayla etkilerini gösterirler (4,5). Bifosfonatlar osteoklastlarin lizozomal enzim üretimini engelledikleri gibi hücre içi fonksiyonlarini da bozarlar. Ayrica prekürsör hücrelerden osteoklast olusumunu inhibe ettikleri gibi, osteoklast apoptozuna da neden olurlar (6).Basta antirezorptif ilaçlarla olmak üzere osteoporozun medikal tedavisinin takibinde, tanida altin standart olarak kabul edilen dansitometrik ölçümlerden faydalanilir. Ancak bu yöntemler biyokimyasal belirteçlere oranla tedaviye verilen cevap hakkinda daha geç bilgi verirler. Bu nedenle antirezorptif tedavinin etkinligini degerlendirmek için kemik döngüsünün biyokimyasal belirleyicilerinin önemi büyüktür. Ayrica kemik döngü hizini saptamak, primer ve sekonder osteoporoz ayirici tanisini yapmak, kirik riski yüksek hastalari belirlemek, tedavi tipini seçmek amaciyla da kemik döngüsünün biyokimyasal belirleyicilerinden yararlanilir (3). Bu belirleyiciler kemik yapim veya yikimini saglayan hücrelerin enzimatik aktivitelerini, kemik yapan hücrelerin enzim olmayan ürünlerini, yapim ve yikim sirasinda serbestlenen kemik matriks komponentlerini spesifik olarak gösterirler (3,4,5,6,7).Biz bu çalismada kemik yikimini engelleyen bir ilaç olan risedronatin postmenapozal osteoporozda, Dual Enerji X-Ray Absorbsiyometri (DEXA) ile degerlendirilen kemik mineral yogunlugu (KMY) ve biyokimyasal belirteçlerden osteokalsin ve idrar C-telopeptidi (CTX) üzerine olan etkilerini arastirmayi amaçladik. Ayrica uygulanan antirezorptif tedavi ile biyokimyasal belirteçlerdeki ilk 3 aylik degisimle, ilk iki yilda gelisen lomber vertebra ve femur proksimalindeki KMY degisimleri arasindaki iliskiyi arastirmak istedik.
